I have just added Emmanuel Press and Lutheran Liturgical Prayer Brotherhood to the list of links. Emmanuel Press has published some excellent little volumes, including the marvelous Brotherhood Prayer Book (BPB). You can check them out by clicking on the link. The most recent offering of EP is a reprint of the early 20th century book: The Explanation of the Common Service.
